Patient Responsibility Guidance Sheet
Patient Expectations at Your Next Appointment
We ask that if you are not feeling well, please stay home.
We are doing expressed check-in which means we review all of your data over the phone before you come in for your appointment. This may be done a few days before or when you arrive to your appointment from your cell phone in your car. To prevent delays in the office, please provide us with all your insurances, vision and medical, both front and back of the card, name and date of birth of the primary insured as well as the patient, prior to the date of your exam. Send to emailus.sec@gmail.com or fax to 570.489.8702.
Call us from your car when you arrive at the office. We will call or text you when everything has been cleaned and it is ready for you to come in. Our number is 570-489-8733. If you do not have a cell phone, call when you are leaving your home so we can get the office ready for you. (Our office door will be locked even though we are open.)
You will be asked to arrive at the office wearing a mask and wear it for your entire visit and there will be a screening before entry into the office.
We are limiting the number of patients in the office. Therefore arrive at your appointment by yourself. Minors can be accompanied by one adult.
Plan to get everything you may need at one visit. Be prepared to order glasses and your annual supply of contact lenses to limit exposure back into the office.
We are offering free shipping on all contact lens orders. We have a new simple way for you to order contacts with a few quick clicks. We are offering curbside pickup of eyeglasses and contact lenses as well.
We are reminding staff and patients to clean and wash hands before and after their exam. We will be spraying your hands with Pure and Clean Disinfectant which is safe and non-caustic.
